Customized Individual Programs:
Legacy Care Coordination and Lifestyle Management has the ability to serve all clients we work with and in the event that there are individuals we are unable to serve we will collaborate with our community partners to meet their need. Our clients, due to their uniqueness, will have different needs whether supportive or independent as they deal with a transitory period in their life. Due to Legacy’s sensitivity in service delivery, we strive to meet the needs of our clients through our creativity in an efficient, comfortable and successful manner.
In order to meet the need of those we serve an initial assessment will be conducted and the information will be gathered from family members and significant others (as appropriate). This process is done face-to-face collaboratively with the client and listening skills are most valuable in this process, as we will be getting to know the individual. With the approval and agreement of the client, the family member may assist in the plan development process as the goals and objectives are outlined. When distance, a quarantine protocol or another obstacle makes a face-to-face visit difficult, our assessment can be accomplished virtually (zoom, skype, face time,) as well.

This customized care plan affords Legacy to partner with our clients to focus on what is most important to them NOT US and achieve the results and support they initially wanted in a time that works for them. Some of our clients utilize our services for a few months and some for a few years. Once all of the set goals are achieved, we will usually contact our client and a family member and let them know that we are discharging our services and that we are no longer needed. This is the biggest compliment to ourselves. It means we did what we set out to do in our quest to serve. Some clients do however choose to keep Legacy Care Coordination on service in order to maintain what was accomplished. In this case we will still perform our visits but more infrequency. We will continue to monitor our client and ensure that gains are not lost.
