Our two founders have a combined six decades in the health service field. Most of our Coordinators have either a Master’s degree or equivalent in a health related discipline. We also have Registered Nurse advisors as well as licensed social workers  (LCSW) and geriatric care managers that are certified with the Academy of Certified Social Workers  (ACSW).

Physicians not returning calls is one of the most common complaints children have and is considered one of the biggest barricades to understanding their parents care needs.  Legacy has many solutions to this. We can attend a medical appointment with your parent’s (with their permission and HIPPA release) and report to you directly or put you on the phone with their doctor (with the doctor and your parent’s permission). If your parents are willing, we can also arrange for them to sign a health care information release (HIPPA) form and present that to the physician’s office so he/she can legally discuss their diagnoses, prognoses, treatment plans and medications with you.  There are many strategies we have to support your parents by empowering them and those who love them get the tools they need to work effectively and navigate around the healthcare system.

We absolutely do and more often then not. We also work with some guardians as well. It is always intelligent to get a fresh opinion and a new perspective from a new set of eyes. We are glad to work with anyone who has his or her loved one’s best interest at heart. We can also write supplemental reports for guardian’s petitions and other court cases.

I feel uncomfortable having strangers come to my Mom and Dad’s house. I don’t know who you are. What assurances do I have that they will be safe during that visit?2019-03-02T02:19:18+00:00

It is normal to want to know who is visiting your loved one especially during times of uncertainty and vulnerability. Rest assured and know that Legacy Care Coordination and Lifestyle Management, LLC is an insured company; each member of Legacy’s team of consultants, nurses, therapists and contractors have been thoroughly vetted, have fidelity bonds and have passed a level II background check.

I suspect that a “friend” is financially abusing my mother and my mom won’t share anything about her finances, what can I do?2019-03-02T02:19:00+00:00

First, be glad to know that Florida has set up an abuse hotline that anyone who even suspects abuse can call anonymously and an investigator must start an investigation within 24 hours of the call. You can still choose to have Legacy Care Coordination perform our face-to-face assessment to evaluate other risks you mother may be facing.

What if you cannot make a contact with my loved one after I paid for a face-to- face evaluation/assessment?2019-03-02T02:12:31+00:00

A full refund, minus a nominal surcharge, will be issued to any client we are not able to contact or who does not grant us permission or reasonable access to their residence or temporary location for us to perform our evaluation.

Why can’t I get the list of Medications my loved one is one or other Medical Information pertaining to my loved ones condition?2019-03-02T02:13:15+00:00

Legacy is a HIPPA (Health Insurance Portability and Accountability Act of 1996)

compliant company who respects the privacy rights of your love one. If you didn’t receive any requested medical information, it probably means that your loved one has not yet signed the HIPPA release that is legally required for us to obtain such information or has not granted us permission to share that information with anyone else.

I paid for a report with safety analysis and care plan. How long does it take to get that report?2019-03-02T02:13:58+00:00

Typically it can take up to 7 days to get a type written version of the report. If there are serious safety, or health concerns a verbal report is issued usually less than 24 hours upon discovery of such a concern.

Otherwise a verbal report is offered to all of our clients within 3 days of making our first contact.

If I want Legacy Care Coordination to continue to provide services for my loved one can I?2019-03-02T02:14:25+00:00

Yes! We are in the business of care coordination and care plan execution. The evaluation and care plan creation you paid for was for investigative purposes ; basically to see if there was some need or issue that your loved one had. Our Care coordinator will cover this with you in the verbal report. We will make recommendations of how much time and how many visits it will take each month to support your loved one. Our agreements are ALWAYS month-to-month. With a short 30-day notice you can start and stop and start services again.  Sometimes we will recommend a stop in service when we have accomplished all of our goals for your loved one and they are safe in their environment.

What does the face-to-face evaluation typically entail?2019-03-02T02:18:03+00:00

In short, we try to cover every major aspect of an individual’s life.   With safety as our number one priority, here are just some of the risks we look for when we evaluate:

  1. Nutritional risks – Is there food in the home, is it outdated, is it compliant with a special diabetic, renal etc, diet
  2. At home Safety risks (fire, falls, bathrooms safety, etc)
  3. Medication management risks (over/under dosing, polypharmacy, medication availability)
  4. Financial Risks (double payments, contractor services, financial abuse)
  5. Caregiver service Risks- Caregiver attendance, punctuality, Are the caregivers providing proper performance as agreed?
  6. Spiritual Needs- access to spiritual leaders, places of worship as desired or needed
  7. Medical Advice Compliance. Are physician orders clear and being correctly followed? Are their conflicts in directives between specialists? Are the specialists and the primary care physicians communicating?
  8. Mental Health risks. Depression screening, suicide risks,
  9. Risky relationships. Are the people involved in your loved ones care acting responsibly? Are they charging for services that are not in alignment with reasonable and customary fees? Are strangers or neighbors managing ATM cards, checkbooks, or withdrawing money without reason? Is your loved one signing deeds or transferring property or other assets without their knowledge?

I have a friend that I am concerned about and want to order a face-to-face safety evaluation for. Do I have to be related to the person I am ordering the assessment for?2019-03-02T02:18:29+00:00

No. Blood relation is NOT a requirement to order an evaluation. The main requirement is that the person being assessed must have the ability to consent to the assessment and be able provide us access to their current location (residence, nursing home, hospital assisted living, shelter, etc.)
