Manage Doctor Visits

Studies show that many of our seniors have a difficult time clearly remembering to convey all of their symptoms and dates of occurrences to medical professionals during their appointments. Furthermore, they have an increasingly difficult time remembering post visit instructions regarding pre and postoperative protocol, medication changes or medication discontinuing instructions. These communication gaps can contribute to potential misdiagnoses as well as delays in treatment, improper medication management, exploratory procedures, and well as elective surgeries.

  • Attending: As part of our care plan development, we may recommend that we accompany you, your client or beloved senior to the Primary Care physician (PHP) and specialist appointments to ensure that communication is clear with both parties and that Physician orders are understood and followed.
  • Scheduling: Legacy Care Coordination can also support our clients with planning and coordinating several health care provider appointments, as well as help get the necessary scans and cardiac evaluations and tests necessary for preoperative clearance and more.

  • Reporting: We will also ensure that any medication changes or pre/post-operative instructions are conveyed to all participating specialists, assisted living/Nursing home Management (when applicable) on a need to know basis, upon approval from the Client or client’s Guardian or Attorney-in –fact via a HIPAA release.
