If you feel that you, your loved one, or your client is medically, physically, or emotionally unsafe in their current environment; or you are concerned with other matters regarding their welfare, Legacy Care Coordination can perform a thorough home medical, safety, and environmental evaluation.

Our multipoint evaluation checks for several potential health and safety hazards.
Our evaluation will provide you with recommendations to support your loved one. We can also provide you with a care plan should you request us to support you with some of the issues of concern.

Our Safety evaluation goal is to verify that:

Medication Counts are correct and doctors orders are being followed
Our client is not accidentally over or under-dosing on their medications
Medically prescribed or recommended diets are being followed
An in-home fall risk evaluation is conducted to validate that your loved-one is reasonably safe in their current living environment.
All supportive services (Aides, Therapy, Home Health, etc.) are in place and working as arranged?
Our Client is safe from financial exploitation or abuse.
Our Client remembers to keep their medical appointments.
Our client is not experiencing self-diagnosed depression or other mental health issues.
Their living environment is clean, healthy, and safe.
Our Client is not at risk or in danger with any aspect of their life.
